It's a really, really good idea to put your name somewhere in your submission — not just in the submission agreement, but in the submission itself. Writers, this means putting your name (preferably formatted as you would like your credit/byline to appear in print) somewhere in your script (e.g. title page, credits box, header or footer, or just a note at the top, etc.). Artists, this means making sure your name (as you would like to be credited) is clearly identified and easily visible somewhere in your submission (e.g. margin notes, credits, title page, etc.).
This just helps avoid a lot of potential confusion when your submission is reviewed.
